- Assessments need to be more comprehensive and consider a lot of factors including environmental safety, and symptom manifestations such as sundowning, delirium, and the client's unique social history.
- Care plans need to be very proactive in providing appropriate, meaningful daily activities that stimulate cognition, mobility, and exercise.

Post Stroke Care
The transition to home after a stroke can be a stressful one. Capabilities have diminished, sometimes quite substantially so that significant assistance is needed. Charming Caregivers and Homemakers‘ core goal is to assist our clients in regaining as much of those capabilities as possible.
Our caregivers work in coordination with our clients' doctors and therapists to support consistent physical, mental, and nutritional programs prescribed by these professionals. We can also recommend other professionals after our client’s home health benefits have run out.
